NRT (nicotine replacement therapy) was originally developed to be used by the Swedish Royal Navy to help relieve nicotine cravings when away at sea and was not originally a 'stop smoking' product.
Today NRT is marketed to the general public as a quit smoking method and is the most widely tried (and failed) of all the quit smoking products with the NRT range consisting of patches, gums, lozenges, inhalers and sprays.
All of these products perform the same task which is to reduce nicotine cravings and cigarette withdrawal symptoms by supplying a smoker with nicotine via an alternative means to cigarettes and this is the problem and the reason why NRT was not originally developed as a stop smoking aid and why it is largely ineffective as one.
These products marketed by pharmaceutical companies to profit from nicotine addiction are claimed to double quit smoking success and are promoted as the best way to stop smoking have been found ineffective in many non-industry funded studies:
Quit Smoking Cold Turkey or with NRT - Which is Best
The Facts about Cold Turkey Vs NRT
- A study published in the Annals of Internal Medicine on the effectiveness of extended duration of transdermal nicotine therapy and reviewed by Dr. Siegel (Professor at the Department of Community Health Sciences, Boston University School of Public Health) in a post on his blog, Tobacco Analysis.
In this post, Dr. Siegel claims: "New study shows that even extended Nicotine Replacement Therapy is extremely ineffective; unaided quitting rates are far better" and concludes that of the 568 participants only a few even with continuous use of the nicotine patch for six months were able to remain non-smokers. Of those using the nicotine patch for a full 52 weeks to achieve continuous abstinence only five (0.8%) which is less than 1 in 100 were able to go the full year without smoking a cigarette.

- Research in Australia reporting on the best quit smoking methods and there success rates and published in the May 2006 edition of Addictive Behavior concluded that of the 2,207 former smokers and 928 current smokers involved in the study:
77.2% quit smoking cold turkey, 35.9% quit using nicotine patches, 35.9% quit using nicotine gum, 35.3% quit using a nicotine inhaler and 22.8% quit using Zyban or Wellbutrin.
The conclusion from this research was that the nicotine patch, gum, inhaler and lozenge were ineffective and that cold turkey was almost twice as effective as the alternative methods.
- Other research by [Davidson, M., Epstein, M., Burt, R., Schaefer, C., Whitworth, G. & McDonald, A. {1998}] showed that only 19% of those using the nicotine patch had stopped smoking (81% were still smoking) and the figure had reduced to 9.2% by the six month mark (91% were still smoking).
For those using the nicotine gum to stop smoking, 84% were still smoking at the 6 week mark and 92% were still smoking at the 6 month mark.

- Finally research by Simon Chapman and colleagues at Australia's Sydney University and published in the journel PLoS Medicine reviewed 511 studies on smoking cessation published between 2007-2008 found that the most successful method used by ex-smokers was "cold turkey" with two-thirds to three-quarters quitting without NRT or quit smoking drugs.
Conclusion
NRT products which give nicotine to nicotine addicts to stop them from smoking is the equivalent of giving alcoholics IV drips full of alcohol to prevent them from drinking, it just isn't going to work.
To sum up NRT products, Ellen DeGeneres said it best: "The nicotine patch only works when you put one over each eye so you can't find your cigarettes!"
Overall NRT will reduce your withdrawal but also prolong it, dragging out your addiction for months. Once you stop using these products you go back to square one with the cravings or you falter and go back to smoking. Quit smoking cold turkey on the other hand is like ripping off a band-aid... it's better to get it over with fast!
To stop smoking you need to rid you're body of nicotine altogether. Cold-turkey in hand with coping skills to master addiction, deal with cigarette withdrawal symptoms and avoid relapse has the strongest chance of success.
